Summary
There are five declarations that require signing. These are:
- Probity: Professional Obligations
- Probity: Convictions and disciplinary actions
- Health: Professional Obligations
- Health: Regulatory and voluntary proceedings
- Educational Agreement
The Educational Agreement is signed by both the GPR and the Educational Supervisor. The remaining four are only signed by the GPR.
